Individualized
Needs Assessments
We are
professionally-trained caseworkers and are
available to assist you with determining your
particular needs and the services that are
available to you. We can help you to remain
independent in your own home or help you to
explore alternative living arrangements suited
to your particular needs. We will meet with you
in your own home, or at another place of your
choosing.
Inter-Agency
Referral Services
Oftentimes, the
variety of organizations and agencies can be
confusing and make it difficult to know where to
begin looking for help. After you have discussed
your particular needs with us, we can provide
information to you about each of the various
agencies and organizations which may be able to
help you. We can explain what services are
available and assist you to get the needed
services.
Financial
Assistance Information
While the Town of
Windsor does not administer any long-term
financial assistance programs, there is help
available through the State of Connecticut’s
Department of Social Services. This help can
consist of cash benefits to help pay monthly
expenses such as rent, utilities, clothing and
food. It may also include non-cash help through
the Food Stamp program. We can help you to apply
for benefits and help you with completing the
forms necessary to do this.
Medical Care and Insurance
Programs Information
There are many different
medical and insurance programs available
depending upon your age, financial situation,
the nature of your disability, and other
circumstances. These programs include Medicare,
Medicare D for prescriptions, Medicare
Advantage, Medicaid, Medigap, ConnPACE, ConnMAP,
and others. We can help you to understand the
differences between these programs and to
determine which of them may be of help to you.
In addition, we can assist you with getting more
information about them and applying for
benefits.
Community-Based
Services
The Social
Services staff in the Town of Windsor directly
administers and coordinates a variety of
community-based services. These include programs
which provide services to Windsor residents to
assist them to remain in their homes and to
avoid placement in nursing homes. Programs like
Friendly Visitors, Friendly Shoppers, and Home
Helpers provide help with shopping, doing minor
tasks around the house, or simply having someone
to spend a little time with each week. Other
programs give you the opportunity to help others
with their needs. We can help you to receive
these services, or to help you to become more
involved in your community.
